Create questionnaire form
Open the new questionnaire form
Open Questionnaires and click New questionnaire to start a reusable form.
- Use the title recruiters will recognize when selecting a questionnaire from an applicant record or bulk action.
- Choose Company for a shared questionnaire or User for a personal questionnaire if your access allows it.
Set delivery and expiration rules
Decide how long questionnaire links stay available, whether applicants can return and edit later, and which channels CareerATS can use to send the questionnaire.
- Set Form links expires in hours. Reminders must fit inside this expiration window.
- Set Time allowed to edit the form only when applicants should have a shorter editing window than the full link expiration.
- Enable Email and choose an email template, or leave the default simple template selected.
- Enable SMS and choose an SMS template when applicants should receive the questionnaire by text.
Build the questionnaire fields
Use the Builder to add fields in the order applicants should complete them. The builder stores the structured field definition and the rendered form markup when you save.
- Click Add field and choose the field type that matches the answer you need.
- Expand a field to edit its Label, Type, Field key, Required setting, Description, and Placeholder when supported.
- Use Up, Down, and Remove to keep the questionnaire ordered and clean.
- Click Preview form to review the applicant-facing form before saving.
Configure options and blocking answers
For choice fields, add the answer options applicants can select. Use blocking carefully when an answer should flag or block the applicant from continuing normally.
- Add option rows for select, radio, or checkbox-style choice questions.
- Keep option labels readable for recruiters and option values stable for saved responses.
- Mark an option as Block only when selecting it should block the applicant.
- For checkbox fields, use Block if checked or Block if unchecked when a single checkbox answer should create the block.
Add a reminder campaign
Use Reminder campaign to send follow-up messages until the questionnaire expires. Reminders are optional; leave the campaign empty if one send is enough.
- Click Add reminder for each follow-up message.
- Set Hours after previous send. Reminder 1 is measured after the initial questionnaire; later reminders are measured after the previous reminder.
- Choose a Send condition: not started, started but not completed, or always.
- Choose Email, SMS, or both, then pick the matching reminder templates or use the default reminder text.
- Watch Reminder total so the combined reminder delay does not exceed the form expiration hours.
Save and test the form
Click Create form, then send the questionnaire to a test applicant before using it broadly.
- Confirm the form appears in the questionnaire list with the right visibility.
- Send it from an applicant record or bulk applicant action to verify delivery settings.
- Open the applicant Questionnaires tab to confirm sent status, expiration, responses, blocks, and pending reminders.
